Before filling the signup form please read the following:-

• One who have their authorised Registration number and Registred clinic only those Doctors are able to fillup the signup form.

• Those Doctors which are banned by the government are not able to fillup this signup form .

• After verifying the Registration number and other details,only then Doctor will be added in Medicamp.
